Short answer
Use QuickBooks Online Plus as the general ledger and your practice-management platform as the subledger, connected by a sales summary journal entry rather than a per-transaction feed. Boulevard documents a native daily journal-entry sync that books gift cards and account credit as liabilities; Vagaro documents a per-transaction sync at $10 a month; Mangomint, Aesthetic Record, Zenoti, Mindbody and AestheticsPro publish no sales-to-QuickBooks sync, so their daily or monthly summary is posted by hand. Either way, package, membership and gift card liabilities must be reconciled to the POS outstanding-balance report every month.
Key takeaways
- QuickBooks Online Plus ($140 a month list) is the right plan for most med spas because it adds class and location tracking that Simple Start and Essentials lack; Advanced is for four or more locations.
- Treat the POS as the subledger and QuickBooks as the ledger: revenue, tax, tips, gift cards, packages and net deposits come across as one summary entry whose lines tie to POS report columns.
- Boulevard is the only platform among those reviewed whose help center documents a daily journal-entry sync (two entries built from its Sales and Payments and Refunds reports); Vagaro documents a per-transaction invoice-and-payment feed.
- Mangomint's QuickBooks connection is payroll only, Aesthetic Record's Learning Lab has no QuickBooks article, and Zenoti, Mindbody, AestheticsPro and Nextech do not publicly document a sales sync, so plan on a manual summary entry.
- No vendor documents sending a package rollforward to QuickBooks; reconcile gift card, package and membership liabilities to the POS outstanding-balance report at every month-end.
- Leave QuickBooks inventory tracking off for injectables: no integration sends unit relief, so count monthly in the POS or EMR and book cost of goods sold from the count.
In this guide
Every med spa already owns two systems that count money: the practice-management or POS platform where patients check out, and whatever the accountant files the return from. The real question is which one is the general ledger, which one is the subledger, and how the numbers travel between them without turning package deposits into revenue or losing the tips in the bank feed.
Our answer: QuickBooks Online Plus is the ledger, the POS is the subledger for patients, items and prepaid balances, and the bridge is a sales summary journal entry, daily if your platform builds it and monthly if it does not. Below is what each platform's QuickBooks connection does according to the vendor's own help center as of October 16, 2026.
Which QuickBooks Online plan a med spa needs
Plus. On Intuit's pricing page it lists at $140 a month with five users, class and location tracking (capped at 40 on that plan), inventory tracking and budgets. Simple Start ($38) and Essentials ($85) have no class or location tracking, which a two-site practice or an MSO structure will miss within a quarter. Advanced ($340) allows unlimited classes and locations and 25 users; it earns its price around four or more locations.
Two settings matter more than the plan. Separate service lines with income accounts, not classes: injectables, energy devices, skincare services, retail, membership fees. A POS summary maps to accounts far more cleanly than to classes, and our chart of accounts guide has the list. Use QuickBooks locations for physical sites; Boulevard's setup documentation tells multi-location practices the same thing.
Leave QuickBooks inventory tracking off for injectables and supplies. QuickBooks inventory assumes every purchase and sale flows through an item record, and none of the integrations below documents sending unit relief. Keep injectable counts in the POS or EMR, count monthly, and book cost of goods sold from the count, as the inventory guide describes.
The ledger, the subledger and the bridge
The POS knows things the ledger never should: which patient owns which package, how many units remain on a voucher, who tipped whom. The ledger needs totals by account that tie back to those reports: the outstanding-balance report supports the liability accounts, sales by category supports revenue, and the payments report supports what reaches the bank.
The bridge is a summary entry. A per-transaction sync that pushes every checkout into QuickBooks as its own invoice creates thousands of documents a year, each needing a customer, an item and a payment match, and it still cannot tell QuickBooks that a package sale is a liability. A summary entry books one day (or one month) in a dozen lines that each tie to a column on a POS report. That is what Boulevard's native integration does, and what we build by hand for everyone else.
What each platform's QuickBooks connection actually does
Where a vendor publishes nothing, the table says so; get anything a salesperson promised in writing.
| Platform | QuickBooks Online sync | What the documentation says moves | What is not documented |
|---|---|---|---|
| Boulevard | Native paid add-on; requires the Premier, Prestige or Enterprise plan (setup article) | Two journal entries each morning from the Sales and Payments & Refunds reports: sales by category, gift card and account credit sales to liability accounts, new membership sales, redeemed vouchers, discounts, refunds, tax, gratuity, card fees and net card deposits | Packages; inventory; the QuickBooks plan required. Boulevard says it fits practices that do not pre-sell injectable units in bulk |
| Vagaro | Native add-on at $10 a month; a Xero add-on at the same price (activation article) | Daily at 2 a.m.; each transaction posts as an invoice or a payment, with an item created for every service, product, class, membership and package; sales tax as a single item; optional "sync transactions after fees are deducted"; the prior two months load at setup | How tips, gift cards, packages and memberships are recognized; inventory. The Xero article lists "sales transactions, taxes, tips, and refunds" |
| Mangomint | QuickBooks Payroll Sync only | Payroll data only. Reports cover sales, payments, gift cards, outstanding packages, memberships, client account balances and inventory | Any sales or revenue sync; the integrations page lists Shopify, Mailchimp, webhooks and clinical tools |
| Aesthetic Record | None; a Learning Lab search for QuickBooks returns no article | A Daily Sales Summary with gross and net sales, a deposit slip and payment methods; the Package Sales report shows the remaining wallet balance as a liability; a Zapier "new invoice" trigger sends date and amount only | Any accounting sync |
| Zenoti | Not documented on the public help center or integrations page we could read | Package and membership revenue rules, gift card settings, register closure, tax configuration | Whether and how sales reach QuickBooks |
| Mindbody | No QuickBooks listing in the marketplace pages we could view; a bookkeeping partner is listed | Sales and liability reports for export | Any native accounting sync |
| AestheticsPro | None listed on its integrations page | Webhooks for invoices and purchases; reports download to Excel | Any accounting sync |
| PatientNow / Nextech | PatientNow shows a QuickBooks logo under its Essentials tier ("sync financial workflows with QuickBooks"); the Nextech pages we read mention no QuickBooks connection | Nothing specific published | Granularity, liabilities, tips, tax, fees, inventory |
The pattern is plain. One platform documents a summary journal entry that books gift cards and account credit as liabilities. One documents a per-transaction feed that creates an item for every package but does not say how the package is recognized. The rest give you reports. In every case a person still reconciles the package liability monthly, because no vendor documents sending a package rollforward to QuickBooks.
A one-day sales summary entry, worked
An illustrative single-location practice in Tampa closes Tuesday, October 13, 2026. The POS daily summary shows $6,400 of services delivered, of which $600 was paid with credits from packages sold in earlier months; $900 of retail skincare; a $1,800 six-session laser package sold today; $500 of gift cards sold; and $420 of tips. Florida does not tax cosmetic or medical services, so sales tax applies to the retail only: 6% state plus Hillsborough County's 1.5% surtax on $900 is $67.50. Patients paid $300 in cash and $9,187.50 by card.
| Account | Debit | Credit | Ties to |
|---|---|---|---|
| Merchant clearing (card payments in transit) | $9,187.50 | Payments report, card total | |
| Cash on hand | $300.00 | Payments report, cash total | |
| Deferred revenue: packages (redeemed today) | $600.00 | Package redemption or voucher activity report | |
| Service revenue (split by line from sales by category) | $6,400.00 | Sales by category, services | |
| Retail product revenue | $900.00 | Sales by category, retail | |
| Sales tax payable | $67.50 | Tax collected column | |
| Tips payable to staff | $420.00 | Gratuity column | |
| Gift card liability | $500.00 | Gift card sales column | |
| Deferred revenue: packages (sold today) | $1,800.00 | Package sales report | |
| Totals | $10,087.50 | $10,087.50 |
Revenue for the day is $7,300, not the $9,487.50 collected. The $2,300 of gift cards and the new package is owed to patients, the $420 belongs to staff and the $67.50 belongs to Florida. The $600 redemption moves out of the package liability and into service revenue on the day the laser was fired, the only day it belongs there.
Two business days later the processor pays out. At 2.75% the fee on $9,187.50 is $252.66, so the entry is a debit to the bank of $8,934.84, a debit to merchant processing fees of $252.66 and a credit to merchant clearing of $9,187.50. The clearing account returns to zero, the bank feed matches a deposit the books already expect, and nothing is counted twice. If your processor deducts fees monthly instead of per batch, debit the bank for the full $9,187.50 and book the fees from the statement.
Tips are neither revenue nor a pass-through to ignore. Under IRS Topic 761, reported tips are subject to income tax withholding and to Social Security and Medicare tax on both sides, and a mandatory service charge is non-tip wages. So the $420 clears through payroll, not through a transfer from the owner's phone.
Reconciling the prepaid liabilities every month
Each liability in that entry (gift cards, packages, and memberships billed in advance) needs a month-end rollforward: opening balance plus sales, less redemptions, less expirations or forfeitures under your written terms, equals closing balance, which must equal the POS outstanding-balance report. Boulevard has Gift Card Balance, Voucher Balance and Account Credit Balance reports; Mangomint has Outstanding Packages, Gift Card and Client Account Balances reports; Aesthetic Record shows the remaining wallet balance on its Package Sales report; Vagaro's Sales Summary lists package and gift card sales and redemptions as separate rows. If your platform cannot show the closing balance by patient, that is a software defect to fix before the next close.
For tax, a cash-method practice reports package money when received; an accrual-method practice may defer an advance payment to the following year under IRS Publication 538, but no further. Keep the management books on the delivered basis regardless. The memberships, packages and gift cards guide covers the policy choices, and Florida gift cards cannot expire.
Xero, and when it is the better choice
Xero works for a med spa. Its U.S. plans list at $27, $59 and $97 a month, inventory is an add-on, and Vagaro's Xero add-on costs the same $10 as its QuickBooks one. But Boulevard's integration is QuickBooks Online only, and most U.S. accountants who serve aesthetics practices run on QuickBooks. Choose Xero if your accountant already lives there; otherwise the ecosystem argument wins.
What usually goes wrong
A sync that disconnects without notice, so three weeks of sales never arrive. A daily sync whose account mapping was never finished, so new POS items land in an unbalanced or uncategorized account. The bank feed accepted on top of the sync, doubling revenue. Package sales mapped to an income account because the setup wizard offered one. Tips mapped to revenue and then paid out as an expense. And QuickBooks inventory turned on for neurotoxin with nobody relieving units, producing a balance sheet that shows $80,000 of product in a refrigerator that holds $11,000.
When to get help
If you are choosing a platform, bring the accounting question to the demo: ask to see the QuickBooks mapping screen and the outstanding-balance report, not the booking calendar. If a sync is already running, pull one day's QuickBooks entry and the same day's POS summary and compare them line by line. If they disagree, or package sales are sitting in income, the fix is a cleanup project before the next month of data makes it worse.

Is QuickBooks Online Essentials enough for a single-location med spa?
It can hold the books, but it has no class or location tracking and no inventory module, so the moment you open a second site or add a management entity you will be migrating. Plus costs $55 more a month at list price and removes that ceiling. For a practice with one location and no MSO, Essentials works for the first year; we still set most clients up on Plus from day one.
Should I let my POS sync every transaction into QuickBooks?
We prefer not to. A per-transaction feed creates thousands of invoices and payments a year, each needing a customer, an item and a bank match, and it still cannot tell QuickBooks that a package sale is a liability rather than revenue. A daily or monthly summary entry carries the same totals in a dozen lines that tie to the POS report. If your platform only offers per-transaction, use it for bank matching and post the liability adjustments by journal entry.
How do I record a package sale in QuickBooks?
Credit a deferred revenue liability account for packages, not an income account, on the day the package is sold. When a session is delivered, debit that liability and credit service revenue for the value of the session. At month-end the liability balance must equal the POS report of outstanding package credits by patient. The same pattern applies to gift cards and prepaid memberships.
My POS has no QuickBooks integration. What do I do?
Export the sales summary, the payments-by-type report and the outstanding-balance reports at month-end and post one summary journal entry from them, following the worked example above. Match merchant deposits through a clearing account so processing fees are booked separately. This takes a bookkeeper about an hour a month once the mapping is written down, and it produces cleaner books than most native syncs do.
Should I turn on inventory tracking in QuickBooks for Botox and fillers?
No. QuickBooks inventory expects every purchase and sale to run through an item, and none of the POS integrations reviewed documents sending unit relief, so the QuickBooks quantity would be wrong within days. Keep unit counts in the POS or EMR, count on the last day of the month, value at cost, and book cost of goods sold as opening inventory plus purchases less closing inventory.
